Obstruction Site and Bronchodilator Reponse in Methacholine-induced Asthma

Abstract
To determine the most effective bronchodilator in methacholine-induced asthma lung function tests were performed in 20 asthmatics before and after the administration of fenoterol or ipratropium. Prebronchodilator values of ?Vmax50 were above 20% in 15 cases(75%), which suggested that the predominant obstruction site is central airway. Nonresponder(?Vmax50 < 20 %) showed severe obstruction indicating that the obstruction site is peripheral airway in severe bronchospasm.
Fenoterol was significantly more effective than ipratropium in methacholineinduced asthma. However, there were no significant differences in the density dependence changes with either agent. The correlation between ¥ÄVmax50 and ¥Ä¥ÄVmax50 was significant(r=-0.90, p<0.001). These data suggested that the obstruction site are more important than the kind of bronchodilator in determining the bronchodilator site.
